10.2. Legal vs. Non-Legal Testing

Legal testing requires verified ID, documentation, and chain-of-custody procedures.
Non-legal (“peace-of-mind”) tests are for informational use only and not admissible in court.

10.3. Test Accuracy & Limitations

All tests are processed by accredited laboratories using validated methods.
While laboratory testing is highly accurate, Samplify Onsite Services LLC is not liable for errors caused by lab processing, specimen tampering, or client-provided information.

10.4. Results Release

Test results are released only to the authorized party listed on the consent form or as required by law.

Clients are responsible for providing accurate contact information for results delivery.

10.5. Payments & Refunds

• Payment is due at the time of service or booking unless otherwise arranged.
• Deposits are nonrefundable once collection services are completed or a kit is issued.
• Refunds are considered only for cancellations made at least 24 hours in advance.
• Once specimens are collected and shipped to the lab, no refunds can be issued.

10.6. Cancellations & Rescheduling

Clients must provide at least 24 hours’ notice to cancel or reschedule an appointment.
Missed appointments without notice may forfeit any deposit paid.

10.7. Courier Services

Medical courier services include pickup, transport, and delivery of medical specimens or materials.

All courier activity follows HIPAA and chain-of-custody compliance. Samplify Onsite Services is not liable for delays or third-party handling beyond our control.

10.8. Liability Disclaimer
